Situation
A U.S. distributor had imported frozen dumplings from the same Korean manufacturer for over five years without issue, and sales kept growing. All the importer had on file was the manufacturer's HACCP certificate and a Korean sanitation certificate — no English FSVP documentation existed.
Problem
Two consumers who ate the dumplings were hospitalized with food poisoning, and health authorities began tracing the source. FDA told the importer, 'Submit your complete FSVP file within 24 hours' — but there was no hazard analysis or supplier evaluation record at all.
Timeline
